A: You were randomly chosen from everyone who receives Social Security disability benefits (SSDI/SSI) and are between the ages of 18 through 64. This means you’re eligible for Social Security’s Ticket to Work (Ticket) Program.
A: The survey asks about your awareness of the Ticket Program, your experience receiving services from an Employment Network (EN) and your work experience.

Your responses will contribute to the most accurate, up-to-date information about awareness and participation in the Ticket to Work Program.

To make sure the results of the survey represent everyone eligible for the Ticket Program, it is very important to hear from everyone who received the survey invitation.
A: Social Security’s Ticket to Work Program supports career development for people ages 18 through 64 who receive Social Security disability benefits (SSDI/SSI) and want to work.

A: Yes! Your answer will help us learn more about people who have not heard of the program or who have chosen not to use an EN.
A: No, the survey will NOT affect your SSA benefits. Your SSA benefits will not change because of your answers, nor whether you complete the survey.
Your participation is voluntary.

Once at the website, enter the personal identification number (PIN) listed in your invitation letter to begin the survey.

If you do not want to complete the survey online or do not have a computer, smartphone, or tablet, you can still participate. To set up a time to complete the survey over the phone, call 1-855-205-3782 (toll-free) Monday through Friday, 9 a.m. to 8 p.m. ET.. If you are deaf, hard of hearing, or have a speech disability, use 7-1-1 to access Text Telephone (TTY) Telecommunications Access Services.
A: Social Security is conducting the survey. Social Security oversees the Ticket to Work Program.

Westat, Inc., a survey research firm, is under contract to collect data for the survey.
A: Social Security and Westat will
  • Protect your identity to the fullest extent allowed under the law.
  • Secure your personal information in a protected electronic file.
  • Combine your answers with answers from other beneficiaries in the study and secure them in a protected electronic file.
  • Identify your answers by a number only.
Social Security and Westat will not:
  • Connect your answers to your name.
  • Put your personal information in any report about this survey.
