Social Security is administering a survey to gather feedback on how satisfied you are with your Employment Network (EN).
You are not required to complete the survey. Individual answers will not be shared with Social Security and your benefits will not be affected if you choose not to reply to the survey. Survey materials will be mailed on September 16, 2016, and responses must be received by November 30.
The EN Beneficiary Satisfaction Survey asks you to:
Social Security randomly selected more than 30,000 beneficiaries to participate. If you were selected, you will be sent a postcard (enclosed in an envelope) with instructions for completing the survey online. If you are not able to complete the survey online, you will receive a paper survey along with a postage-paid envelope for its return.
If you receive a survey and have questions or need assistance completing the survey, call JBS International at 1-800-839-5561 or email ttw@jbsinternational.com. JBS International will be conducting the survey on behalf of Social Security. TTY users, please contact your local Relay Center for assistance if you need to reach JBS by phone.
