Beneficiary Satisfaction Survey: How is your Ticket to Work Employment Network Doing?

Social Security is administering a survey to gather feedback on how satisfied you are with your Employment Network (EN).

You are not required to complete the survey. Individual answers will not be shared with Social Security and your benefits will not be affected if you choose not to reply to the survey.  Survey materials will be mailed on September 16, 2016, and responses must be received by November 30.

The EN Beneficiary Satisfaction Survey asks you to:

  • Rate how satisfied you are with your EN’s services and staff
  • Identify your EN’s strengths and weaknesses
  • Evaluate how well your EN meets your needs and expectations
  • Describe your job, if you are working

How do I know if I’ve been selected?

Social Security randomly selected more than 30,000 beneficiaries to participate. If you were selected, you will be sent a postcard (enclosed in an envelope) with instructions for completing the survey online. If you are not able to complete the survey online, you will receive a paper survey along with a postage-paid envelope for its return.
