Information collected in this manner is used solely for responding to requests for information or records. We may forward your email to other government employees who are better able to respond to you; however, we will not share any of your personal information with any other organization or individual. Section 205(a) of the Social Security Act, as amended, authorizes us to collect the information requested on this form. Social Security will only use your contact information to register you for a WISE webinar and to provide you with related Ticket to Work Program information that may benefit you. The Privacy Act (5 U.S.C. § 552a(b)) permits us to disclose the information you provide on this form in accordance with approved routine uses. Giving us this information is voluntary; however, if you do not complete the required fields we cannot provide you with these services. Additional information regarding this form, routine uses of information, and other Social Security programs, is available on our website, www.ssa.gov, or at your local Social Security office.

Finally, the Privacy Act of 1974, as amended (5 U.S.C. 552a), requires us to notify you that the Privacy Act protects the information you are providing to us. If you sign up for email updates we will send you information about the Ticket to Work Program and other Social Security benefit or program information that might be of value to you. You may unsubscribe from Ticket to Work Program updates at any time by emailing the word UNSUBSCRIBE in the body of an email addressed from the email account from which you signed up to support@choosework.ssa.gov.

Text messages will require an additional confirmation that you want to receive them, and instructions on how to stop them will be provided when you confirm your subscription. Standard text messaging rates will apply.
